Upon our recent installation of the cone beam 3-D imaging technology developed by Sirona, Dr. Marinic participated in a Chicago area television production of a Medical and Dental Health program. Dr. Marinic outlines the advancements and new accuracy standards in treatment that surpass the once acceptable standards of old 2 dimensional xrays and radiographs.

These CAT Scans totally eliminate the guess work on determing what parts of the jaw bone offer the best sites for implant placement.

Many doctors however don't possess the techology to accurately drill into an implant site, thereby creating, in some situations, cantilevered implants. The restoration process, or placement of the crown, can accommodate an angled implant... but the implant may cause other oral health problems later.

CatScan for Nobel Biocare Guide To overcome the possibility of creating a poor placement, Nobel Biocare has created a CAD/CAM engineered device called the Nobel Guide that is created from the primary CAT Scan, as described above.

This first photo in our series of photos outlining the Teeth In an Hour process indicates severe deficits in upper jawbone (maxillary) material.

Unfortunately this patient had numerous problems caused by two previous dentists who had improperly placed the implants which were further compromised by improper implant lengths, poor choice of abutments and spans between implants being too long. Eventually, practically all of the procedures and materials failed.

Chicago Teeth in one Hour The adjacent photo reflects the outward appearance of the patient's previous treatment. The prosthetic device is broken and held together with denture adhesive. Parts of the jawbone were destroyed and were causing the patient constant chronic pain.

Implants, when properly implemented, provide an ideal method for distributing the forces of bite and mastication (chewing). Because of the differences in the amount of biting forces that operate on posterior and anterior teeth, poor placements or an insufficient number of implants will lead to further complications.

Implant Failure Long Spans Our next photo shows the condition of our patient's upper jaw (maxilla) and indicates the previous implant placements. The lack of symmetry in the overall condition of the maxilla is also apparent.

Several implants had to be removed. In addition, the remaining tooth structure was severley decayed which resulted in pain and eventually removal. The precision is generally 200 micron. This level of precision can not be achieved by placing implants “freehand.”

A temporary prosthesis was created that conformed to the bite and occlusal issues of the patient.

Denture CatScan Nobel Implant Guides This step facilitates the CAD/CAM engineering aspect of determining the most appropriate placement, drilling depth and type of implant for each implant site.

Virtual Dentistry via CAD/CAM Engineering

The next photo illustrates the virtual placement of the implants. The surgeon has the flexability to change the position, quantity , type, length and diameter of the implant.

Computerized Imaging Nobel Implant Guides
In addition to verifying that there is adequate bone surround the implants and that there is a safe zone around all vital structures. Restoratively the location of implants are determined to verify that they are in a proper position.

After the final position of the implants has been determined and verified, the file containing the Nobel Guide information is sent via e-mail to Nobel Biocare in Sweden.

Within two weeks the precision Nobel Guide surgical template is returned. A highly skilled dental laboratory uses this guide to create an exact replica of how the jaw will look after the implants are placed. This same guide is sterilized and used during the surgical procedure.
