Loose Denture Diagnosis and Treatment in ChicagoCauses - Effects - Treatment OptionsDentures that have a poor fit, either at the outset of treatment or after years of usage, become loose for specific reasons.The factors that cause or infuence this common problem of denture wearers determines what treatments will be worthless and which will create the elimination of the problem. Immediate Poor FitBrand new dentures that are sized and fabricated in low cost denture clinics tend to have the highest rate of treatment failure. Low cost dentures, by definition, are focused on maintaining the least amount of cost for easy affordability.Extraction surgeries, existing gum and periodontal tissue conditions and the state of bone loss and/or jawbone height can interfere with the anticipation of having a denture fit perfectly and without adjustment problems (adjustment of the prosthetic directly or adjustment of the patient). Preliminary Causes
Denture treatment provided through a general, restorative or cosmetic dentist can be expected to adequately diagnose a patient's existing and "anticipated" oral health through xrays, catscans (if warranted) and the basic exploratory examination. Loosening Dentures within the Short TermNew false teeth that undergo multiple adjustments and/or relines can reflect good efforts at creating an accommodation but may indicate underlying conditions that may warrant specific treatments.Custom relines and extensive use of denture adhesives do a credible job of making nearly any denture product comfortable, at least for the short term. Uneven bone height, tissue density and jawbone issues are essentially masked. Unfortunately, the tendency for us, as dental patients, to use pain or the absence of pain as a reliable indicator of treatment need has a tendency to create even more problems. As an example, it is not uncommon for patients to assume they have gum disease because nothing hurts. The absence or controlled perception of pain and discomfort can easily mask or cover up underlying conditions that can accelerate the loss of important bone mass, density, height and overall gum tissue health that is critical for maintaining a reasonable fit of dentures. Dentures that float, rock back and forth, slide around or fall out can easily cause rapid deterioration each time an occlusal bite is forced ... for eating and chewing. Excess movement, slipping, popping causes wear and tear of immediate tissues (gums) and underlying structures to be subjected to forces that shouldn't be operating (Imtec, Sterngold, Intra-Lock mini implants have become popular stop-gap measures for this). Loose Denture RepairWith the possible exception of retrofitting or modifying an older denture into an implant retained prosthesis (create mounts for mini implant anchors), there isn't much that can be done to a denture that functions poorly. For some patients, obtaining a better, more accurate, impression may be that all is required.Denture fabrication processes used by dentists are typically more accurate and have significantly less shrinkage than those processes used by large denture clinics. Shrinkage alone can be a determing factor for some patients. Patients who have forced themselves to endure a fit that seems to be worsening may need to be assessed for jawbone augmentation, soft tissue grafts or, in some cases, an alveoplasty, to smooth out or flatten uneven areas. Looseness: Long Term Denture UsePatients who have worn dentures for a long, long time (15 years or more) have normal experiences of loose denture fit for the common reasons of wear and tear. Denture products that provide the best fit and are maintained faithfully by denture wearers will inevitably become loose.In an ironic sense, for patients who have worn dentures for too long (yes... there is indeed a limit) their vigilence in promoting good oral health can lead to bone loss that is so severe that a new fitting for dentures becomes impossible. More and more long term denture wearers, while maintaining ideal denture function throughout the years, have unknowingly caused an inperceptible, but steady, loss of bone. Since pain, discomfort, sloppy fit or rocking dentures typically don't appear, this patient group is discovering that the use of dentures can be lost as a treatment choice for maintaining the ability to eat, chew, smile and speak without special effort. Some patients with long term hard and soft tissue loss may be candidates for specialized, hybrid tooth replacement treatments.
