Reconstructive DentistryAge Specific Definitions of Full Mouth Reconstructive TreatmentReconstructive and/or Full Mouth Makeover Dentistry means different things to different people, depending on what the patient knows about different procedures and, oddly enough, certain age variables.Television programming during the past few years has done much to educate consumers about many cosmetic dentistry procedures and products (e.g., Extreme Makeover show). Within a short amount of time, cosmetic or restorative treatments including crowns, bridges and especially porcelain veneers became associated with "makeover treatments." Synonyms in DentistryAdditional language and terms have become associated which can often lead to confusion and miscommunication about what a patient asks for and receives. Smile makeover, extreme makeover, full mouth reconstruction, reconstructive makeover, restorative dentistry are phrases that are used almost interchangably by some.Young Adult Reconstructive MakeoverGenerally speaking, younger patients tend to be more cosmetically conscious. Common wear and tear factors haven't accumulated to the degree that an older, middle aged adult or senior may have. A young patient's makeover (more restorative/cosmetic than reconstructive) therefore may tend to be more focused on having straight, symmetrical bright white teeth with a balanced smile line. Such characterstics are known to create an alluring smile that creates a makeover experience that can do wonders for self esteem.Creative combinations of porcelain veneers, crowns, teeth whitening, tooth colored fillings, amalgam replacement and the popular gum contouring or gum reshaping treatments are the mainstay of true cosmetic dental makeovers. Older Adult Reconstructive MakeoverDental reconstruction for the older adult, although possessing some or all of the same characteristics of his/her younger counterpart, requires considerations of more basic structural factors. Existing condition of soft tissues, gums, connective tissue, bone mass, bone integrity, loss of vertical dimension, occlusal abnormalities (potential or existing) are almost routinely and automatically included in a makeover consultation by the tenured reconstructive dentist.Failure to evaluate certain structural factors, especially for older patients, has the potential of causing premature failure of "makeover" (here, it refers to restorative) products or procedures. We have all heard stories of common dental crown treatments causing pain and tmj problems. Too often, the remedy of grinding down new restoratios to relieve the problem are used. In other instances, treatments of brand new veneer products turning strange shades of gray or chipping and cracking and even falling off have become too well known. Treatment failures are rarely caused by the dental product used (veneer, crown, bridge, etc). Rather, an underlying condition or constellation of conditions undermines the cosmetic result. Dental crowns or veneers, for example, that cause pain, breakage or bleeding gums typing suggest an occlusal factor that was factored incorrectly. Fillings and crowns that fail (fall out or leak) tend to be caused by deep seated bacteria issues that are more common among older patient populations. In a similar fashion, overbite and underbite issues for younger adults can exist or be caused by entirely different sets of circumstances than those experience by an older adult due to the dynamics of bone health, current wear and tear and the almost assumed existence of some degree of periodontal disease (75% of adults have some form of periodontal disease). Assumptions for Treatment LongevityBeing mindful of the numerous issues that involve normal wear and tear and the tendency for patients to accumulate one or more dental issues can assure getting the most sensible treatment that endures.
