Comprehensive Reconstructive Implant DentistryBone GraftingIn some cases prior to placing an implant, the site needs to be augmented to allow for adaquate height, width and quality of bone. This is done through bone grafting, or using special types of implants.If teeth require extraction, it is imperative that they are removed with minimal trauma to the remaining bone. Many times grafting could have been eliminated if the bone surrounding the extracted teeth was protected. Types of Bone Grafting MaterialsSelf donated (autogenous) can be harvested from the lower jaw, illiac crest, calvarium (skull), rib and tibia. Although autogenous grafts are considered the "gold standard" of grafting, it involves an additional surgical site.An osseous coagulum trap can be used to save bone during an implant placement. This bone is excellent for grafting an immediate implant placement. Human source (allograft) Bone Banks may be demineralized freeze dried bone, freeze dried bone matrix, irradiated bone, fresh frozen bone and human bone ash. When combined with Platelet Rich Plasma (PRP) these materials serve as an excellent graft for sinus lifts and ridge-splitting techniques. Species other than man (xenograft) include bovine porcine are readily available. CeraMed adds Pepgen P-15, a synthetic biomimetic of the 15 amino acid sequence of Type I collagen, that is responsible for cell binding. This results in more rapid bone growth. Artificial synthetic bone materials (alloplasts) can be ceramic or glass. In some cases synthetic bone grafts can actually retard natural healing mechanisms and cause immuno-inflammatory reactions. Membrane GraftsMembranes are used to prevent the soft tissue from migrating into the graft material before the bone has a chance to grow.Nonresorbable membranes require an additional surgery to remove, but can stay longer in the graft site and in some cases are more predictable. Resorbable membranes dissolve by themselves after a few months which eliminates the need and additional cost for surgery. Types of graftingOnlay grafts are used to increase the width or height of a particular implant site. Generally the bone is self donated. Several tissue banks are now offering block grafts to be used with PRP that can promote and accelerate tissue healing.Sinus lifts - by type:Caulldwell Luc: Surgical entry is made from the side of the upper jaw. Grafting materials may be allograft or xenograft with PRP.Osteotome technique: Surgical entry to the sinus can occur at the crest of the ridge (where the teeth used to be). This procedure nonetheless has limitations, depending on the condition of the ridge. In general terms this type of entry is much less traumatic when it is possible to perform. Ridge splitting: A thin ridge on the upper jaw in the front with adaquate height can be split. Allograft or xenograft material can be used, along with PRP to faciliate healing. This procedure increases the width of the ridge. In some cases the implants can be placed immediately following the graft procedure.
